As part of New York State’s Medicaid 1115 Waiver, the Food Bank delivers nutrition services to eligible Medicaid members, including produce prescriptions, pantry stocking, and nutrition counseling and education. Nutritious, medically tailored groceries are delivered directly to neighbors’ homes on a weekly basis, helping to reduce barriers to access and support improved health outcomes.

Frequently Asked Questions
We offer Food Prescription Boxes, Pantry Stocking Boxes, and Nutrition Counseling services through Medicaid. Nutrition counseling includes either four 30-minute sessions or eight 15-minute sessions. Participants may also receive both food box services and counseling
To be eligible for services, recipients must be Medicaid beneficiaries who meet the program’s eligibility requirements. Recipients cannot be utilizing other medically tailored meal or food prescription box services at the same time. To find out if you are eligible, visit healthyalliance.org. Services are typically provided for up to six months, and recipients may be reassessed at the end of the service period for potential continuation of services.

Each box includes a variety of rotating food items, along with a packet containing nutrition education materials and recipes related to the foods provided. Your first box will also include a folder with program information and contact details for our Program Manager in case any questions arise.
Medicaid Prescription Boxes include 4–5 pounds of protein items focused on lean meats, seafood, and plant-based proteins; 2–3 pounds of whole grains such as whole wheat pasta, quinoa, and brown rice; 10 pounds of fresh produce sourced from a local farm; and additional pantry staples.
Pantry Stocking Boxes include canned proteins, canned fruits and vegetables, grains, and 5 pounds of fresh produce sourced from a local farm.
Yes! During intake, you will be asked about any cultural or religious preferences, allergies, dietary preferences, or health conditions so your boxes can be tailored to your needs. If there are any changes to your health status or disease management, you can contact our Program Manager to adjust future boxes in real time.
We ship directly to homes throughout our entire service area, including the North Country, Capital Region, and Hudson Valley. Please refer to our service map if you have any further questions.
